Hepatic VOD / SOS definitions (Seattle, Baltimore, EBMT 2016)

Reports which of three published definitions of hepatic veno-occlusive disease a case meets.

Example

Days since transplant
12
Hepatomegaly or right upper quadrant pain of liver origin
Yes
Weight gain from baseline
Weight gain above 2% but not above 5% of baseline

Result: Modified Seattle is met and the other two are not

The tool opens with these values already filled in. Replace them with your own.

What the result means

Modified Seattle
Within 20 days, two of three: bilirubin above 2 mg/dL, hepatomegaly or right upper quadrant pain of liver origin, weight gain above 2% of baseline.
Baltimore
Within 21 days, a bilirubin at or above 2 mg/dL plus two of three: painful hepatomegaly, ascites, weight gain above 5%.
EBMT 2016 classical
Within 21 days, the Baltimore items.
EBMT 2016 late-onset
Beyond day 21: the classical picture, or histological proof, or two or more classical items with hemodynamic or ultrasound evidence.
Where they part
Baltimore and the 2016 criteria count nothing until the bilirubin is raised; modified Seattle counts it as one of three, so a normal bilirubin can meet one and neither of the others.
Day 21
No longer an exit. The late-onset category was added because disease beyond the classical window is real.

What you enter

  • Days since transplant (every definition is written around this count)
  • Bilirubin at or above 2 mg/dL
  • Hepatomegaly or right upper quadrant pain of liver origin (the modified Seattle wording)
  • Painful hepatomegaly (the Baltimore and EBMT wording; it satisfies the broader item above as well)
  • Ascites
  • Weight gain from baseline
  • Hemodynamic or ultrasound evidence of veno-occlusive disease (a late-onset route beyond day 21)
  • Histologically proven (a late-onset route beyond day 21)
