- Low risk.
- 3 to 4
- Intermediate risk.

- Sex item
- ONE DIRECTION ONLY: female donor into male recipient. Not a generic "sex mismatch" item.
- Donor
- Only two published categories. HAPLOIDENTICAL AND CORD-BLOOD DONORS HAVE NO DEFINED VALUE.
- Timing
- 12 months or less = 0. Some reproductions print "<12 / >12", leaving exactly 12 unclassified.
- Aplastic anemia
- ALWAYS scores 0 for stage, by definition - the ladder is built from remission states it lacks.
- Scope
- Group-level pre-transplant estimate. Does not decide whether to transplant. A high score is NOT a reason to withhold it.
- Axis
- Scores the disease and the transplant. The HCT-CI scores organ comorbidity. Complementary, not alternatives.

- Disease stage [early = 0; intermediate = 1; late = 2]. Severe aplastic anemia is always early.
- Transplanted in first complete remission. YES SUPPRESSES THE TIMING ITEM ENTIRELY: it scores 0 regardless of the interval, and the maximum reachable score becomes 6.
- Months from diagnosis to transplant. Required ONLY outside first complete remission. More than 12 scores 1.
- Donor type [hla-identical-sibling = 0; unrelated = 1]. THESE ARE THE ONLY TWO PUBLISHED CATEGORIES - haploidentical and cord-blood donors have no defined value in this score.
- Female donor into a male recipient. ONE DIRECTION ONLY: this exact combination scores 1; male donor into female recipient and both matched combinations score 0.
