ICANS Neurotoxicity Grade (ASTCT)
ASTCT consensus ICANS neurotoxicity grading (Lee 2019) after immune-effector-cell / CAR-T therapy.

Example
- ICE score
- 1
Result: ICANS grade 3 of 4 (severe), driven by the ICE score domain.
The tool opens with these values already filled in. Replace them with your own.
What the result means
- 1
- ICE score 7-9; no worse consciousness, seizure, motor, or raised-ICP finding.
- 2
- ICE score 3-6 and/or awakens to voice.
- 3
- ICE score 0-2, awakens only to tactile stimulus, a resolving seizure, or focal edema on imaging. Severe — typically ICU-level.
- 4
- Unarousable, prolonged/repetitive seizures, deep focal motor weakness, or diffuse cerebral edema / raised ICP. Severe.
What you enter
- ICE score (0-10)
- Level of consciousness
- Seizure
- Raised ICP / cerebral edema
- Deep focal motor weakness
