What this is

Estimate glomerular filtration rate using three published equations side by side: CKD-EPI 2021 (the race-free equation now endorsed by NKF and ASN), MDRD (the legacy four-variable equation many older studies report), and Cockcroft-Gault (which estimates creatinine clearance rather than GFR and is still the FDA reference for many drug-dosing labels). Sophie Well takes serum creatinine, age, sex, and weight, then returns all three estimates so a clinician can see how the choice of equation changes the number for a given patient.

When to use it

Use this when reviewing a patient with chronic kidney disease and a recent creatinine, when a drug label specifies a CrCl threshold (vancomycin, DOACs, gabapentin, sotalol) and the EHR is reporting eGFR instead, or when staging CKD against the KDIGO categories. Note: the CKD-EPI 2021 equation no longer uses a race coefficient - that was the explicit point of the 2021 update - so any prior NKF/ASN-aligned chart eGFR computed before the 2021 switch may not match what Sophie Well returns.
