What this is
Look up renal dose adjustments for commonly prescribed antibiotics. Sophie Well covers the agents whose FDA labels include explicit CrCl- or eGFR-based dose-reduction tables - vancomycin, piperacillin-tazobactam, cefepime, meropenem, levofloxacin, ciprofloxacin, trimethoprim-sulfamethoxazole, gabapentin (frequently bundled with antibiotic regimens), and the rest of the common renal-adjusted list. Enter a patient's CrCl or eGFR and the agent, and the tool returns the dose, the interval, and the threshold the label crossed to get there.
When to use it
