What the result means
- Histological
- An endomyocardial biopsy showing non-caseating granuloma with no alternative cause. Definite on its own; nothing else is needed.
- Clinical
- ALL THREE: a histological diagnosis of extracardiac sarcoidosis; one or more qualifying cardiac findings; other causes reasonably excluded. Yields probable.
- Cardiac findings
- Steroid-responsive cardiomyopathy or block; unexplained LVEF 40% or less; unexplained sustained VT; Mobitz II or complete block; cardiac FDG-PET, MRI or gallium uptake.
- Caution
- Not meeting the criteria does not exclude the disease - endomyocardial biopsy misses patchy involvement often. Decides nothing about immunosuppression or a defibrillator.
