Weight-Based Dose

Weight-Based Dose - a deterministic tool in Sophie Well's Medication & Infusion group.

What this is

Compute a total dose from a per-kilogram dose and a patient weight using the standard formula total = weight (kg) * dose per kg. Sophie Well accepts mg/kg, mcg/kg, units/kg, and mL/kg, returns the rounded total, and surfaces the underlying math so the bedside calculation is auditable.

When to use it

Use this for any weight-based medication order - pediatric antibiotics, heparin bolus, anesthesia induction agents, naloxone, dexamethasone, vasopressors, and so on. The output is arithmetic only; you still have to verify against the institution's formulary and any per-dose maximum (a pediatric weight-based calculation that exceeds the adult ceiling should be capped, not delivered).

References

Standard weight-based dose: total = weight * per-kg dose. Verify against your formulary.

Worked example: Total 350 mg
