What this is
Generate a print-ready, wallet-sized medication and health summary - the patient's name and date of birth, allergies (NKDA emitted by default when the allergy field is empty, since unknown is not the same as none documented), the current medication list with doses and frequencies, key conditions, the care-team contacts (primary care, key specialists, pharmacy), and an emergency contact. Sophie Well formats it as a compact, two-column card the patient can fold and carry.
When to use it
Use this at any visit where medication reconciliation is the limiting step (new specialist, ED visit, post-discharge follow-up, pre-op clearance), or at the request of an older patient or caregiver who wants a stable copy that does not depend on the portal being up or the chart being current at the receiving facility. The card supplements, but does not replace, a pharmacist-reviewed medication list - the patient maintains it, and "the medications they actually take" is sometimes a different list from "the medications their chart says they take". Update it after any med change and at every appointment that touched the regimen.
