What this is
Sophie Well takes the patient weight, sex, measured sodium, chosen infusate (3% saline, 0.9% NS, 0.45% NS, D5W, or oral free water), and a 24-hour sodium target, then returns the expected change per litre of that infusate and the rate needed to hit the target without exceeding the conventional 8-10 mEq/L per 24 h ceiling. The output flags rates that would overcorrect and reminds you that ongoing urine losses can shift the plan within hours.
When to use it
Reach for this any time you are about to write a correction order for symptomatic or severe dysnatremia and want to sanity-check the rate before it goes in the pump. It is a planning aid, not a substitute for hourly sodium re-checks - osmotic demyelination from overcorrected hyponatremia and cerebral edema from over-correction of chronic hypernatremia are both iatrogenic and both happen when teams stop measuring. Pair the planner with serial Na, urine output, and fluid-balance checks every 2-4 hours and reset the trajectory when reality drifts from the plan.
