What this is
Fill in the four SBAR fields - Situation (one sentence: who is the patient and what is the problem right now), Background (relevant history, admission reason, current treatment, and the trajectory), Assessment (what you think is going on and why), and Recommendation (what you want the receiver to do, and by when) - and Sophie Well returns a clean copy-paste handoff card. The format is the IHI-standardized communication scaffold developed for Navy submarine communication and adapted for healthcare by Kaiser Permanente in the early 2000s.
When to use it
Use this for any structured clinical handoff where the receiver was not in the room: nurse-to-provider escalation about a deteriorating patient, shift-change handoff, transfer of accepting service, OR-to-PACU, ED-to-floor, or any phone call to a consultant or rapid response. SBAR's job is to compress the relevant context into the fewest words a receiver can act on - the Recommendation field is the one trainees most often skip, and it is the one that turns a handoff into a decision. Specific, time-bound asks ("please come evaluate within 15 minutes", "please order a CT now") outperform vague ones ("can you see this patient").
