What this is

A templated patient question-generator for an upcoming healthcare appointment. Sophie Well asks the visit type (new diagnosis, follow-up, procedure, second opinion, mental-health visit, pre-op, post-discharge, etc.) and the visit context, then returns a printable list of questions to bring to the visit - what is the diagnosis, what are my options, what are the trade-offs, what happens if I do nothing, what should I watch for, who do I call after-hours. Question scaffolds are drawn from the AHRQ "Questions to Ask Your Doctor" patient-engagement library.

When to use it

Use this before a visit you are nervous about, a visit where new treatment decisions are likely, or a visit where the previous one ended with "call us if anything changes" and you want a structured way to report what changed. The questions are starting points, not a script - the highest-yield use is to bring three or four that match the specific decision the visit will hit, and to leave room for the clinician to lead the encounter. Patients who arrive with written questions are better remembered, ask more follow-ups, and leave with better recall of the plan.
