What the result means

MASLD
Hepatic steatosis with at least one of five cardiometabolic criteria, and alcohol below the MetALD band.
MetALD
The same, with an alcohol intake of 140 to 350 g/week in females or 210 to 420 in males. A category created in 2023.
ALD
Alcohol above the MetALD band.
Cryptogenic SLD
Steatosis with no cardiometabolic criterion and no other identified cause.
Positive, not exclusion
The old term required ruling out significant alcohol first. This one does not, and drinking above the threshold does not remove the diagnosis - it renames the category.
The five criteria
Adiposity by BMI or waist; dysglycemia or type 2 diabetes; blood pressure 130/85 or its treatment; triglycerides 150 or more; low HDL. Any ONE suffices.
BMI is ancestry-specific
The cut is 25 generally and 23 in those of Asian ancestry. Waist cuts vary by ancestry AND sex, with the Japanese thresholds inverting the usual sex ordering.
HDL is sex-specific
At or below 40 mg/dL in males, 50 in females. One figure for both misclassifies a predictable group.
Alcohol is per WEEK
The published bands are grams per week. The daily figures in circulation are approximations of them.
