What the result means
- Step 1
- Confirm excess adiposity: direct body fat measurement, OR one anthropometric criterion plus body mass index, OR two anthropometric criteria regardless of it.
- The exception
- Above a body mass index of 40, excess adiposity can pragmatically be assumed and no further confirmation is required.
- Index alone is not a diagnosis
- Below 40 a body mass index on its own confirms nothing. The Commission recommends it as a population-level surrogate for risk, not as an individual assessment.
- The reverse miss
- Two anthropometric criteria confirm obesity REGARDLESS of the index, so a normal index with a raised waist and waist-to-height ratio qualifies. An index-first screen never reaches that person.
- Clinical obesity
- Confirmed adiposity WITH signs or symptoms of reduced tissue or organ function due to it, or a substantial age-adjusted limitation of day-to-day activities.
- Preclinical obesity
- Confirmed adiposity with preserved organ function, and a generally increased risk of progressing.
- Function, not size
- A higher body mass index does not make obesity clinical. Nothing in the second step reads a number.
- Cutoffs
- The Commission specifies validated cutoffs appropriate to age, gender and ethnicity rather than publishing one set, so none are encoded here.
