What this is
A bedside cross-reference between the four-point AVPU scale (Alert, responds to Voice, responds to Pain, Unresponsive) and the 15-point Glasgow Coma Scale. AVPU is the rapid screen taught in BLS, EMS field assessment, and the rapid-response trigger lists on most inpatient floors; GCS is the granular score used for ED, trauma, and ICU documentation. Sophie Well shows the conventional AVPU -> GCS bands (A ~ 15, V ~ 12-13, P ~ 8, U ~ 3) and notes that the mapping is an approximation, not an exchange rate.
When to use it
Use this when handing off a patient between two care environments that document mental status differently - a floor nurse who tagged a patient "P on AVPU" to a rapid-response team that wants a GCS, or a prehospital crew that called in an "unresponsive" patient to an ED that will chart a full GCS on arrival. The conversion is a translation aid, not a substitute: anyone who is going to act on the number - intubate, image, escalate - should re-score with the actual scale and document each component (eye / verbal / motor for GCS). A drop of two or more GCS points or any worsening on AVPU is the actionable signal.
