Open the tool → Runs in your browser. Data leaves only if you deliberately send a problem report from the interactive tool.

Example

Claim
$200.00 billed; $120.00 paid
Adjustments
$50.00 contractual and $30.00 patient responsibility

Result: 1 of 1 claims balance; 1 of 1 payments reconcile. Patient responsibility: $30.00.

Answer the questions this way to reach that result. Your own answers give your own.

Inputs and output

What you enter

One or more X12 835 version 5010 text files, up to 50 MB each. The reader accepts the file's declared separators, including tilde or newline segment terminators.

What you get
A payment and claim balance summary, a claim-level preview, and full or redacted CSV downloads. Redacted downloads mask patient names, member IDs and patient account numbers.

What this is

The reader checks every service line, every claim and the payment total, including provider-level PLB adjustments, while preserving the raw adjustment codes.

When to use it

Use this after a payer or clearinghouse sends an electronic remittance advice and you need a claim spreadsheet, patient-responsibility totals or an exact location for an out-of-balance amount. Files remain in the browser tab.
