What the result means
- Unit
- THE PUBLISHED UNIT IS ONE ANATOMICAL REGION. The patient total is the SUM of regional scores. A regional score is not the total.
- Region
- 3 points for the region being involved.
- Nodules
- 1 point each.
- Fistulas
- 6 points each. A DRAINING FISTULA IS WORTH SIX NODULES: type dominates count, so six nodules and one fistula score the same.
- Distance
- TRIPLES at each step: under 5 cm 1; 5 to 10 cm 3; over 10 cm 9. A single span over 10 cm is worth NINE nodules.
- Separation
- 9 points if lesions are NOT separated by normal skin. That is the HURLEY STAGE III criterion restated, so hurley-stage is not independent of this score.
- Maximum
- THERE IS NONE. Lesions are counted individually and regions summed, so the score is unbounded. Never report "x of y"; never normalize it.
- Bands
- WITHHELD. One reproduction gives high above 60, moderate 20 to 60; a review states no bands exist. Single-sourced, so reported not applied.
- Provenance
- Superseded for being time-consuming in extensive disease. The IHS4 came from a Delphi process to give an easy-to-use formula.
- Scope
- Examination findings only. Measures extent at one point in time. Does not diagnose or select treatment.
- Burden
- Does not measure pain, drainage, odor or quality of life, so a falling score does not by itself mean the patient feels better.
