- Gestational age [0 - 36 weeks or more; 1 - 32 weeks to 35 weeks 6 days; 2 - 28 weeks to 31 weeks 6 days; 3 - less than 28 weeks]
- Behavioral state before the procedure [0 - active and awake, eyes open, facial movements; 1 - quiet and awake, eyes open, no facial movements; 2 - active and asleep, eyes closed, facial movements; 3 - quiet and asleep, eyes closed, no facial movements]
- Maximum heart-rate rise from baseline [0 - 0 to 4 beats per minute; 1 - 5 to 14 beats per minute; 2 - 15 to 24 beats per minute; 3 - 25 beats per minute or more]
- Minimum oxygen-saturation fall from baseline [0 - 0 to 2.4 percent; 1 - 2.5 to 4.9 percent; 2 - 5.0 to 7.4 percent; 3 - 7.5 percent or more]
- Brow bulge [0 - none, less than 10 percent of the time; 1 - minimum, 10 to 39 percent of the time; 2 - moderate, 40 to 69 percent of the time; 3 - maximum, 70 percent of the time or more]
