- Thrombolysis or embolectomy needed
- Active bleeding or high bleeding risk

- PE diagnosed while on anticoagulation
- Severe pain needing IV analgesia > 24 h
- Medical or social reason for admission > 24 h
- Creatinine clearance < 30 mL/min
