- Under 50 = 1; 50-199 = 2; 200-800 = 3; over 800 ng/L = 4.
- NT-proBNP
- Under 300 = 1; 300-649 = 2; 650-1100 = 3; over 1100 ng/L = 4. TAKES PRECEDENCE over BNP.
- Gaps

- WHO functional class. Optional. NOTE THE LADDER STOPS AT 3: no class scores 4 [I-II = grade 1; III = grade 2; IV = grade 3]
- Six-minute walk distance. Optional. Over 440 = 1; 440 to 320 = 2; 319 to 165 = 3; under 165 = 4. A value strictly between 319 and 320 falls in a GAP and is refused.
- NT-proBNP. Optional, and TAKES PRECEDENCE over BNP when both are supplied. Under 300 = 1; 300 to 649 = 2; 650 to 1100 = 3; over 1100 = 4. A value between 649 and 650 falls in a GAP.
- BNP. Optional, and IGNORED if NT-proBNP is supplied. Under 50 = 1; 50 to 199 = 2; 200 to 800 = 3; over 800 = 4. A value between 199 and 200 falls in a GAP.

- Method
- Grade each available variable 1-4, take the MEAN, round to the NEAREST integer.
- Denominator
- The number of variables AVAILABLE, not a fixed 3. A patient with 2 of 3 is scorable.
