What this is
Score the five-component Bishop score for cervical readiness before labor induction: dilation (0-3 points), effacement (0-3), station (0-3), cervical consistency (0-2), and cervical position (0-2), for a total of 0-13. Sophie Well returns the total with the standard interpretation: a score of 8 or higher predicts a vaginal delivery probability comparable to spontaneous labor; a score of 6 or lower (often described as an 'unfavorable cervix') is associated with longer induction times and higher cesarean rates and typically prompts cervical ripening before oxytocin.
When to use it
Use this when planning a labor induction or when counseling a patient about expected induction success. The Bishop score is most useful for nulliparous patients - in multiparous patients with a history of vaginal delivery, even lower scores are commonly successful. Modified scoring schemas exist (modified Bishop, simplified Bishop) but the components are the same; use the version your hospital protocol references for documentation. The score guides ripening agent selection (mechanical vs. prostaglandin) and counseling but does not change the indication for induction.
