Berlin definition of ARDS (Ranieri 2012): timing within 1 week, bilateral opacities, an origin not fully explained by cardiac failure/overload, and PEEP >= 5 must all be present.

- moderate
- ARDS, moderate: PaO2/FiO2 100-200 on PEEP >=5 (Berlin 2012).
- severe
- ARDS, severe: PaO2/FiO2 <=100 on PEEP >=5 (Berlin 2012).
