  • First-generation antihistamine (diphenhydramine, hydroxyzine, promethazine, chlorpheniramine)
  • Tricyclic antidepressant (amitriptyline, doxepin >6 mg, imipramine, nortriptyline)
  • Skeletal-muscle relaxant (cyclobenzaprine, methocarbamol, carisoprodol)
  • Benzodiazepine (any)
  • Z-drug hypnotic (zolpidem, zaleplon, eszopiclone)
  • Antipsychotic (first- or second-generation)
  • Long-acting sulfonylurea (glyburide, chlorpropamide)

  • Proton-pump inhibitor (chronic, >8 weeks)
